Camp Physical Forms Update

2009 will be a transition year for Physical Forms within the BSA. The old Class I II and III forms will become obsolete on January 1, 2010. The new form is available in the CDB Forms and Publications tab as well as the link below. For 2009 the Class I II form will be accepted at Camp Daniel Boone for base campers. Class III will be accepted for High Adventure Treks. The new form will also be accepted in 2009. For 2010 only the new form will be accepted. It is suggested that any scout getting a physical this year use the new form.

Physical Forms should NOT be mailed in. They should be brought with each troop and presented to the camp nurse at check-in.

Is it true that you will accept the old version medical forms that are, of course, less than one year old?
Also, do you need the originals or will copies suffice?

we will accept last years form so long as it is not expired. We will accept original forms only.

Thank you for your answers. One last, hopefully, question.
A couple of families who have just joined our troop form other troops have the old forms without the originals. Is the need for the originals absolutely inflexible? they just wanted me to ask.
